How do you bring a variety of actors together to address community level measures across multiple diseases and settings? The Pueblo Triple Aim Coalition (PTAC) has created a strategic plan that contains recommendations to address community level issues through a portfolio of projects that directly impact overall community health. This presentation will discuss the development of PTAC, its goals are to improve health, reduce the per capita cost of care and improve the experience of care in Pueblo County, otherwise known as the "Triple Aim", through the use of the concept of Collective Impact to engage and enable high level community actors.
